This study was designed to compare between endoscopic and open surgical repair techniques in treatment of zygomatic fractures. The variables evaluated were age, gender, sites of fracture, treatment options and postoperative complications. During the period from January 2011 to June 2013, thirty Egyptians patients presented with zygomatic fracture. From which fifteen patients were treated by open surgical repair and the other fifteen patients were treated by endoscopic repair in Sheikh Zayed Specialized hospital .To accomplish this technique, a rigid 4-mm, 30-degree down-angled endoscope was used. zygomatic arch was approached endoscopically through two or three small incisions on the transoral or temporal scalp. The duration of follow-up ranged from 6 months to 15 months. Endoscopic repair was proven to give ideal aesthetic results for facial skeleton with smaller scar and fewer complications.
