Abstract

The universally accepted treatment modality of intertrochanteric and subtrochanteric fractures is open/closed reduction and internal fixation using sliding hip screw devices and intramedullary fixation systems as the Gamma-nailHowever, these fractures commonly occur in a geriatric population well known to suffer from a high rate of peri-operative complications and associated co- morbidities resulting in a high rate of morbidities and mortalities when treated traditionally.A much smaller patient group constitutes the young active patient who sustained a high energy trauma to the proximal femur. Often the orthopedic surgeon is confronted with a highly comminuted and/or open fracture in a poly traumatized patient with a vague prognosis.We evaluated external fixation in 38 intertrochanteric fractures and 12 subtrochanteric fractures in a prospective study. The patient’s age ranged from 17 years to 80 years. Overall the functional outcome varied from good to excellent as assessed by the Harris Hip score.Dynamic external fixation of trochanteric and subtrochanteric fractures is a simple, safe and reliable treatment option especially in the fragile elderly population and complex open fractures caused by firearms.
