Abstract

BackgroundBreast reduction is a frequently performed procedure and one with high patientsatisfaction,however the multitude of modifications indicates that the ideal techniquehas yet to be found. A thorough understanding of the complications of breastreduction and how to avoid them can also be very helpful. Caldwell L. et al., 2000Few patients however present for revisional procedures and even fewer for asecondary or repeated reduction mammaplasty. These procedures may range fromminor surgical debridement, flap advancement, scar revision for hypertrophic scar tosecondary nipple areola reconstruction or redo reduction. Rohrich R. F. et al., 2003It is generally recognized that secondary breast reduction should use the same pedicleas in the original procedure; however, more recent reports suggest that using adifferent pedicle may be “safe”; foremost, if information regarding the initialprocedure is unavailable. Loose J. et al., 2000This study was conducted between Between May 2009 and September 2011 with theintention of categorizing the types of revisional surgeries and choosing a specificmethod for each patient.MethodologyThe study was conducted on 30 cases presenting the different types of complications,Between May 2009 and September 2011.,the study was held in Kasr Al Aini hospitaland a specific surgical plan was devised for each individual patient. Patients were thenfollowed up in the early post operative period for complications and at least for oneyear after surgery, we used reduction, mastopexy , nipple and areolar reconstruction ,and scar revision.ConclusionCategorizing patients and conducting a tailored plan for each patient was the key tosuccessful outcome. The study concluded that no technique is superior over the other and that there are guidelines to be followed to achieve patient satisfaction.
