Abstract

This study was carried out to evaluate the effects of sildenafil in perioperative management of parturient with pulmonary hypertension undergoing caesrean section.24 parturient subjected to elective C.S were anasthetized with Epidural anesthesia.Incremental doses (5ml) of lidocaine (1%), bupivacaine (0.25%) mixture (up to 20ml) To attain sensory block at T8 level. The patient were randomized to oral sildenafil50mg one hour before anesthesia and then 25 mg 3 times daily postparturn for at least one month in 12 patients while other 12patients will not receive sildenafil (control group) . Patients will be transferred to I.C.U and managed for 3 days.12 will continue on sildenafil (25mg 3 times daily).An epicardial transthoracic echocardiography will be performed postoperatively daily for 3 days then after one month to provide information regarding Pulmonary BPand EF,Hemodynamics parameters,ABGs analysis and Mother and neonatal outcomes will be compared between the groups.In sildenfial group a significant improvement maternal oxygenation, pulmonary BP,EF and maternal outcomes after sildenafil administration. In control group no change in maternal oxygenation or decrease in pulmonary BP or improvement in maternal outcome. These finding suggest that perioperative treatment with sildenafil is significant treatment of pulmonary hypertension in parturient undergoing caesrean section.
