The Ponseti method for clubfeet correction has demonstrated excellent results. However, relapses are common and continue to be the most important problem following treatment. In this study, 14 patients (20 feet) of idiopathic club feet having dynamic supination after initial correction by ponseti method treated by total transfer of tibialis anterior tendon to lateral cuniform. Results: All cases had no dynamic supination after TATT. Sixteen feet (80%) were considered to have a normal appearance except for the surgical scar, only four feet (20%) developed metatarsus adduction. Conclusion: Total tibialis anterior tendon transfer to the lateral cuniform is a safe and effective procedure in treatment of residual dynamic supination of corrected club feet by Ponseti technique.
