Intratympanic therapy has given the phrase “stick it in your ear” new meaning! What is intratympanic therapy? Is it medical or surgical treatment?Is it good care? In this article, the basis and experience for this emerging technique are examined.This essay focuses on the therapeutic use of intratympanic aminoglycosides, steroids and lidocaine in humans and points out some clinical concerns and gaps in our knowledge.The term “transtympanic injection” has been used. “Transtympanic injection” should mean that the injection is performed across “trans” the tympanic cavity, perhaps into the inner ear via the round window. It seems prudent to use the term “intratympanic” injection. The most important forms of intratympanic therapy are the use of aminoglycosides, particularly streptomycin and gentamicin, corticosteroids for meniere’s disease and lidocaine for Tinnitus.
