Abstract

Introduction: Renal disease is a common health problem that is linked to excess morbidity and mortality worldwide. It is crucial to know the etiology and clinical features of renal disease to promote prevention strategies and to implement adequate resources for the management of this entity. Our study aimed to obtain a comprehensive review of the pattern and spectrum of renal diseasesPatients and methods: In this study therefore, we reported the pattern and spectrum of renal diseases among patients presented to the Acute Haemodialysis Unit (King Fahd Unit) in Kasr El-Aini Hospital - Cairo University. We analyzed clinical and laboratory factors associated with overall morbidity in all presented patients who were indicated either laboratory or clinically to receive renal replacement therapy . We also reported the difference between the Intermittent Daily Haemodialysis and Continous Renal Replacement Therapy(CRRT)(CVVHD or CVVHDF) regarding efficacy, clinical and laboratory outcome in haemodynamically unstable patients who developed Acute Kidney Injury in the ICU unit due to different causes. This was in the duration between October 2010 and October 2012.Results: The total number of patients during the study period was 3804 patient who received haemodialysis in the Acute Haemodialysis Unit (King Fahd Unit) this was the first part of study, The mean age of patients included in the first part of the study was 48.15±15.19 (11-89 years). Most of the cases were males forming about 51.86% and the 40 haemodynamically unstable patients who developed Acute Kidney Injury in ICU and received either Intermittent Daily Haemodialysis (16 patients) or Continous Renal Replacement Therapy(CRRT)(CVVHD or CVVHDF) (24 patients)and this was the second part of study.The mean age of patients included in the second part of the study was 51.03 ± 14.568 years (20-70 years). Most of the cases were males forming about 62.5%. The average duration of hospital stay during the study period was 10.1± 3.17days (5-20 days). The spectrum of renal disease in the first part of the study was six groups: A-patients with acute kidney injury represented 17.4% , B- Patients with chronic kidney disease presented with acute rising of serum creatinine represented 1.13% , C-Patients with chronic kidney disease progressed to end stage renal disease for regular haemodialysis represented 13.3%. D-Patients with Chronic Renal Failure on regular haemodialysis presented with another medical or surgical complications represented 33.5% of the study population E-Patients with obstructive uropathy represented 6.99% , F-patients who had the first presentation of their renal disease and discovered to have end stage renal disease for regular haemodialysis represented 27.65 %.The second part of the study had fourty patient most of the cases were males 25 patient forming about 62.5% ,average duration of hospital stay was 10.1± 3.17days (5-20 days),This group was divided into two groups : sixteen patients who received Intermittent Daily Haemodialysis, their average age 50.31 ± 14.089 years (20 –70 years), Nine of them were males (56.25%) ,while Twenty Four cases treated with CRRT the average age was 51.03 ± 14.568 years (20-70 years) ,Sixteen of them were males (66.66%). Out of the Sixteen patients treated with IDH Twelve of them (75% ) recovered clinically and restored partial improvement of kidney function on discharge after Renal Replacement Therapy during their hospital stay while four cases were died (25%) due to associated medical conditions. Most of the survivors in this group were due to different causes of bleeding and received four frequent session as average number of sessions and Among the Twenty Four patients received Continous Renal Replacement Therapy(CRRT) (CVVHD or CVVHDF), Sixteen cases (75%) recovered clinically and restored partial improvement of kidney function on discharge after Renal Replacement Therapy during their hospital stay while eight cases died (25%) due to associated medical conditions. Most of the survivors in this group were due to post CABG. and received two sessions as average number of sessions.
