Correlation of uterine and umbilical Doppler velocimetry, biophysical profile scoring and placental grading with foetal outcome in diabetic pregnancy

Abstract

Objective: To study the uterine and the umbilical blood circulation using Doppler ultrasound, biophysical profile scoring and placental grading and their correlation with fetal outcome in diabetic pregnancy. Design: Prospective randomized trial. Setting: Outpatient Clinic and Emergency Department of Obstetrics and Gynecology, Faculty of Medicine, Cairo University. Patients: 60 pregnant women of comparable age, parity, weight and gestational age. They were divided into 2 groups: The control group comprising 30 normal non-diabetic pregnant women and the normotensive diabetic group comprising 30 pregnant diabetic women. All were singleton pregnancies of 37-39 weeks and were delivered by C.S. Intervention: The control and the study cases were subjected to history taking and thorough physical examination. They were also subjected to ultrasonographic examination for fetal biometric parameters, BPP, placental grading and for Doppler examination. Main outcome measures: Feasibility of placental grading, biophysical profile and Doppler in prediction of fetal outcome. Results: The uterine arteries Doppler indices showed no significant finding between the diabetic and the control groups. Neither did the uterine arteries Doppler indices show a correlation with the mean blood glucose levels. Conclusion In any case of diabetes during pregnancy follow-up by Doppler ultrasonography is recommended even if all other parameters of fetal health were of normal values. However, we cannot expect to depend on Doppler parameter alone in prediction of fetal outcome as there are multiple factors involved indetermination of pregnancy outcome.
