Abstract

The incidence of delayed EDH has recently increased. Diagnosis is based on a high suspicions index as well as performing a second CT. Proper diagnosis can lead to surgical evacuation before the development of neurological deficits. Twenty patients with extradural hematoma, 16 males and 4 females were studied in CAIRO UNIVERSITY HOSPITAL and in ELMAADI MILITARY HOSPITAL. 19 of the 20 patients (95%) showed no EDH and one patient (5%) showed a small hematoma in the first CT. Initial GCS revealed 3 cases (15%) with 9-12 score and 17 (85%) with 12-15 score. High risk minor injuries were positive in all patients. Risk factors were sever shock in 4 (20%) and coagulopathy in 4 (20%). Contributary factors were dehydrating measures in 3 (15%), otorehea in 3 (15%), and rhinorrhoea in 2 (10%). The second C.T. was positive in all the patients. The interval between the two C.T. examinations varied between few hours to few days. The evaluation by (GOS) after 6 months revealed 13 (65%) patients in the good recovery group, 2 (10%) patients in the moderate disability group, one patient (5%) in the severe disability group, one patient (5%) in the persistent vegetative state, and 3 patients (15%) in the mortality group.
