Objectives and Aim of the Work: The specific aim of this study was to study the incidence and type of various infectious problems in early postoperative period in LDLT.Patients and Methods: This is a retrospective study of 25 patients with liver cell failure mostly due to viral cause submitted to living donor liver transplantation (LDLT). Preoperative and early postoperative follow up for one month were done in the form of clinical examination, laboratory parameters, and cultures when there is clinically suspected infection.Results: High incidence of bacterial infections (80% of cases) in early postoperative period mainly Gram positive and the main source was the biliary drains. The risk factors for early postoperative infection were related to presence of preoperative D.M. and SBP.Postoperative pleural effusion, biliary obstruction (stricture) and high immunosuppressive level also considered as risk factors. GRBW and prolonged ICU and total hospital stay were related to the incidence of early post operative infection.Conclusions: The high incidence of postoperative bacterial infection mainly of biliary source in the early post operative periodRecommendations: The high incidence of early postoperative bacterial infection raise the issues of regular and strict follow up of the infection control procedures all through the steps of early LDLT operation.
