Unexplained infertility is still a diagnosis of exclusion that is made when a couple is involuntarily infertile and no abnormalities are revealed by a standard infertility evaluation. The proportion of the infertile couples diagnosed with unexplained infertility ranges from 10% to 15% . Both the age of the female and duration of infertility are important variables that affect the prognosis for pregnancy. The diagnosis of the possible etiologies of unexplained infertility necessitates more sophisticated diagnostic approaches.In the absence of a correctable cause, the therapy for unexplained infertility is, by default, empiric. Various treatment options include expectant management, intrauterine insemination, controlled ovarian hyperstimulation, combined COH/IUI, and the assisted reproductive technologies. The current state of knowledge in the form of the various published data is summarized, assessed, and the evidence is made and the recommendations are graded according to the level of evidence they were based upon. The personal impact of infertility should be taken into consideration.
