Abstract

Objectives: To assess the effect of local anesthetic agent wound infiltration on pain relief after C.S and its side effects. Patients and methods : A prospective randomized control trial was designed to assess the efficacy of 20 ml 0.25 % Bupivacaine and 20 ml 1 % lidocaine hydrochloride wound infiltration compared to the control group. There was ninety full term pregnant female randomized into three groups :- Group A ( 30 patients ) ---- > lidocaine group, Group B ( 30 patients ) ---- > Bupivacaine group, Group C ( 30 patients ) --- > control group.All patients had C.S under general anesthesia and receive postoperative analgesia (pethidine) according to visual analogue scale value. The total amount of pethidine used was assessed at 30 minutes, 2, 4, 6, 8, 12 and 24 hours. Frist time request anesthesia and time of ambulation were assessed. Results: Bupivacaine infiltration and Lidocaine infiltration in the wound before closure of the skin leads to a reduction in the overall consumption of analgesics in the first 24 hours, and also significantly increases the time interval until the first request for an analgesics. There was a significant difference in the VAS especially in the first 6 hours compared to control group. There was a significant difference in ambulation time in group A, B compared to group C. There was a significant difference between group A and group B as regard first time request analgesia and time of ambulation. Conclusion : we concluded that infiltration of the wound of CS with local anesthetic agent ( Bupivacaine and lidocaine ) give effective analgesia for several hours, decreased systemic opioid consumption, decreased ambulation time and prolonged first time request analgesia, this protocol could be applied by all members of obstetrical team for all C.S under general anesthesia. Recommendation: We recommend using local anesthesia as lidocaine or bupivacaine as analgesic in infiltration of caesarean section wound as it reduce consumption of analgesics postoperative ; so, it provides less side effects of analgesics and provides earlier ambulation.
