Prevalence of subclinical central vascular disease in obese Egyptian patients

Abstract

Background Worldwide, Egypt is among the first four countries suffering from obesity. waist circumference [WC] is a component used to define metabolic syndrome[MS]. The role of WC as an independent predictor of silent myocardial ischemia [SMI], above its contribution to metabolic syndrome, remains unknown in Egyptian obese patients hence we aimed to detect SMI in such patients.Methods Fifty obese patients without overt coronary artery disease[CAD] [Group 1; G1: 30 “60%” females and 20 males “40%”; Mean age 40.2 ± 4.5 years] and 20 age & sex matched non-obese subjects served as a control group [G2] were enrolled in this prospective study. For all subjects anthropometric measures were taken. Laboratory assessment included fasting blood sugar, serum triglycerides, high density lipoprotein cholesterol [HDL], serum uric acid, urea, creatinine and Microalbuminuria assay. High WC and MS were defined per the revised National Cholesterol Education Program [NCEP-R]. Tc-99m SestaMIBI gated SPECT myocardial perfusion imaging [G-SPECT] was used for screening of SMI.Results All obese patients belonged to MS [Not diabetics or hypertensive] with WC “116.9 ± 15 cm”. Among the patients’ group, 13/50 [26%] had SMI compared to 1/20 [5%] among the controls [P= 0.0001]. Positive G-SPECT correlated significantly with HDL (r= - 0.4; P= 0.02); Microalbuminuria [r= 0.5; P= 0.04] and WC [r = 0.6; P 0.04]. On multivariable logistic regression analysis a WC ≥ 94 cm (1.065; 0.806-1.1209; P=0.01), MS (1.031, 0.901-1.1156; P 0.02), and Micro-albuminuria (1.045, 0.926-1.1309; P=0.01) were independent predictors of silent CAD. Conclusion Obese Egyptian patients with WC >80cm in women and ≥94 cm in men are independently associated with an increased prevalence of SMI.
