Abstract

Amyloidosis is not a single entity, but there are multiple different types of amyloid fibrils. The kidney is one of the most important major organs involved in the course of all forms of systemic amyloidosis and it is considered a major cause of death. This study sought to study histologically amyloid deposition in the kidney and evaluate its pattern of involvement and associated interstitial fibrosis and also to determine the type of amyloid deposited whether it is AL or AA-type, to be able to correlate between the type of amyloid deposited, the pattern of involvement, fibrosis and the available clinical data. Paraffin blocks of 40 cases of amyloid kidney were included, cut at 4 microns, and stained with hematoxylin & eosin, Masson trichrome, periodic acid-Schiff, and alkaline Congo red before and after oxidation by using the potassium permanganate method.All cases were examined under light and polarized microscopy. The incidence of AA-type (60%) was higher than AL-type (40%). Glomeruli are more prominently affected than any other compartment. AA-type was associated with more interstitial, arterial and tubular deposits. AA-type was associated with more interstitial cellular infiltrates and fibrosis with subsequent more impairment in renal function. The pattern and degree of glomerular deposition showed no difference between both types. The two types showed no difference in the mean level of proteinuria. Proteinuria depends on the degree of glomerular affection, while serum creatinine depends mainly on the degree of interstitial cellular infiltration, amyloid deposition in the interstitium and interstitial fibrosis.
