The aim of this study is to study fasting insulin resistance index (F.I.R.I.) as simple measure of insulin resistance and its correlation with cardiovascular complication. This study was conducted on 60 obese type 2 diabetic patients and 10 healthy volunteers who were subjected to the following investigation. Fasting serum insulin, fasting blood sugar, lipid profile, cardiac enzymes (CPK,LOH), cardiac troponin I and ECG with the following results, There is significant comparison between patients and control group regarding, fasting insulin, F.I.R.I, S. cholesterol, serum triglycerides LDL and cardiac troponin I.Also there is siginficant comparison between patients with high F.I.R.I. and patients with low F.I.R.I regarding cardiac troponin I and ischemic changes on ECG.
