Abstract

C. difficile is an opportunistic anaerobic spore forming bacterium that colonizes the intestine and flourishes after antibiotic treatment characteristically after Clindamycin, Ampicilin, and Cephalosporins.The bacterium is one of the commonest causes of nosocomial infection accounting for 10 % to 25 % of all cases of antibiotic associated diarrhea and virtually all cases of antibiotic associated pseudomembranous colitis. The major virulence factors of C. difficile are the (Entero) toxin A and the (Cyto) toxin B both of which mediate cytoskeletal derangement , mucosal injury and inflammation. The use of TechLab tox. A/B П test (recent improved ELISA) in diagnosis of CDAD was studied and compared with the tissue culture cytotoxin assay regarding sensitivity, specificity, simplicity and cost effectiveness.ELISA and tissue culture assay evaluated 78 stool specimens from suspected cases of CDAD. Out of them 27+ve specimens (34.62%) were detected by ELISA compared to 29+ve specimens (37.20%) detected by tissue culture.10 additional stool specimens were collected from healthy individuals to serve as control group. In which no toxins could be detected by any of the study methods. ELISA recorded a sensitivity of (93.11%) and specificity of (100%), correlation between the two test methods was (97.4%).From this study we can conclude that TechLab toxA/BП is an easy, simple as well as accurate test that can be used in ordinary laboratories for detection of CDAD helping reduce costs of diagnosis.
