The object of the present work is to study the cytomorphological feature and the value of silver colloid staining method in distinguishing follicular adenoma and follicular carcinoma in FNAC smears of thyroid nodules. 50 histologically confirmed aspirated samples of thyroid follicular lesions were stained by Papanicolaou stain and studied cytologically retrospectively. There was much overlapping between benign and malignant lesions making the differentiation difficult and unreliable. These smears were destained and subjected to silver stained method for Ag-NOR proteins. The Ag-NOR dots appeared as dark brown or black dots in nuclei and were counted in 50 randomly selected follicular epithelial cells in every case. The mean Ag-NOR count were statistically significantly higher in malignant cases in comparison to benign cases as P value < 0.05. Qualitative evaluation of Ag-NOR dots was as important as quantitative one.
