Abstract

Secretory diarrhea is a principal cause of infant mortality worldwide and is one of the top infectious causes of mortality with an estimated 5 million deaths annually.Secretory diarrhes occurs when a patho-physiological event may reduce the absorption rate in either of two ways: by stimulating secretion or inhibiting absorption. Secretory diarrhea can result form bacterial toxins, redueed absorptive surface area caused by disease or resection, luminal secretagogus, circulating secretagogues and medical problems that compromise regulation of intestinal function. Insecrebry diarrhea there is little or no response to fasting. Furthermore, stool Na is generally above 90 mMol/L and the difference between stool osmolality and 2x (Na + K +) in stool is less than 50m Mol/L. Possible novel targets for antisecreory therapy ; namely, 5-hydroxytryptamine receptor antagonists, substance P antagonists and sigma-receptor agonists. There is also the possibility for potentiating the proabsorptive effects of endogenous enkephalins by using enkephalinase inhibitors.
