Abstract

Rheumatoid arthritis (RA) is a systemic chronic inflammatoryautoimmune disease that leads to progressive joint destruction, functionaldisability and extra-articular manifestations. Osteoporosis is a progressivesystemic skeletal disease characterized by low bone mass density andmicroarchitectural deterioration of bone tissue, with a consequent increase inbone fragility and susceptibility to fracture. The isokinetic exercise allows themuscle to exert its maximal contraction in all angles of the range of motion .Theintroduction of Isokinetic dynamometers gives an objective method to measuremuscle strength during the whole amplitude of the articular movement. The purpose was To detect the relationship between the rheumatoid arthritis andboth of isokinetic knee muscle evaluation and DEXA scan. Also to detectthe possibility to use both measures as a detector for the disability in RA. 50 RA patients (41 females & 9 males), age (52.52±8.869) yearsand 50 controls (41 females &9 males), age (52.06±7.681).Both groupssubjected to full history taking ,full general and local examination,Isokinetic knee muscle evaluation and DEXA scan. There was a significant difference between the control group andpatient group as regard the peak torque, average power and total work.Also there was a significant difference between CG &RA group as regardthe lumber spineT-score and left hip T-score. There was a moderatesignificant correlation between walking distance and isokinetic kneemuscle evaluation. Also there was a mild significant correlation betweenPDF created DAS & isokinetic knee muscle evaluation. There was mild significantcorrelation between the Lt hip T-score and isokinetic knee muscleevaluation. There was none significant correlation between the IKME andboth ESR, HAQ and lumber spine T-score. Isokinetic knee muscle evaluation and DEXA scan can be used asa measure of disability in rheumatoid arthritis in combination with the other measures of disability.
