Effects of auditing on quality of some aspects of reproductive health services

Abstract

This study is an operational research, conducted in two family health units of primary health care (PHC), in Giza Governorate; El Talbia and Giza FHU. A pre/post comparison was done in a test and a control unit; El Talbia FHU was randomly selected for implementing the clinical audit model. The study aimed at evaluating the effects of implementing clinical audit on performance of reproductive health services in the selected family health unit and assessing client satisfaction, before and after clinical audit among clients attending reproductive health services of the selected family health unit. The study included health providers and clients. All physicians and nurses working in antenatal care (ANC) and Family planning (FP) clinics in either unit were included. Results of the intervention revealed significant improvement of the clinical practices done by health care providers during FP and ANC examination in El Talbia FHU before and after the intervention. There was a significantly higher mean score for general examination (12.70 0.6) and IUD insertion (13.68 0.5) after intervention at the intervention unit compared to the control unit (10.08 0.4 and 12.00 0.1 respectively) with P≤ 0.001. A significant difference was detected between both units after intervention for mean pelvic examination scores (P = 0.02). There was a significantly higher mean score for general examination of FP at the intervention unit compared to the control unit with P≤0.001. The intervention unit showed a highly significant improvement in mean scores for all antenatal services after intervention (25.21 0.5 for examination and 11.63 0.7 for counseling with P ≤0.001). Furthermore, it was significantly better (P ≤ 0.001) than the control unit that remained almost unaffected. It was concluded that training of the providers will be reflected on improvement of reproductive health care providers in many aspects. It is recommended that clinical and management audit should go hand in hand with a system for supportive supervision to detect deficiencies and correct them.
