Abstract

Purpose: The aim of the present study is to evaluate changes in intraocularpressure following intravitreal injections of different doses of bevacizumab.Subjects & Methods: This is a prospective, randomized, comparative clinicalstudy that was conducted on 60 eyes of 60 patients. Patients who werescheduled for intravitreal injection of bevacizumab due to diabetic macularedema (DME), cystoid macular edema (CME), choroidal neovascularmembrane (CNVM), branch and central retinal vein occlusion (BRVO &CRVO) and proliferative diabetic retinopathy (PDR). They were randomlydivided into 2 groups. Group A: 30 eyes were scheduled for intravitrealinjection of bevacizumab 0.1 ml (2.5 mg). The mean age was (59.80±7.27years). Group B: 30 eyes were scheduled for intravitreal bevacizumab 0.05 ml(1.25 mg). The mean age was (59.83±6.29 years). IOP was measured usingGoldmann applanation tonometer before the injection, IOP was measuredbefore and after Bevacizumab injection at 15 minutes, 30 minutes, 24 hours and7 days after injection.Results: The mean IOP before the injection in group (A) was 15.03±2.25 mmHg with range from 12 mm Hg to 19 mm Hg; and in group (B) mean IOP was15.73±2.66 mm Hg with range from 12 mm Hg to 20 mm Hg. Post injection 15minutes the IOP had risen in both groups. In group (A) mean IOP was43.60±6.27 mm Hg with range from 32 mm Hg to 58 mm Hg; and in group (B)mean IOP was 30.16 ±4.19 mm Hg with range from 22 mm Hg to 38 mmHg.IOP30 minutes after injection in group (A) 25.53±6.45 mm Hg with a rangeof 17 mmHg to 38 mm Hg and in group (B) mean IOP was 20.57 ±2.49 mmHgwith a range of 16 mm Hg to 26 mm Hg. One day after the injection the IOPdropped to a mean of 15.33±2.93 mmHg with a range from 11 mm Hg to 20mm Hg in group (A) and a mean of 15.93 ±2.59 mmHg with a range from 12mm Hg to 24 mm Hg in group (B). Follow up one week after the injection, themean IOP dropped in group (A) to 14.20±2.27 mm Hg with a range from 10mm Hg to 18 mm Hg; and in group (B) mean IOP was 15.00 ±2.02 mm Hg witha range from 12 mm Hg to 18 mm Hg.Conclusions: Intravitreal Bevacizumab causes an immediate transient increasein intraocular pressure (IOP) which tends to normalize one day after theinjection.
