Role of prostatic massage in treatment of chronic prostatitis induced ejaculatory ducts obstruction

Abstract

In our study, the main aim is to evaluate the role of prostatic massage in treatment of chronic prostatitis induced EDO. Infertile patients, with low semen volume, pyospermia, evidence of chronic prostatitis and EDO as evidenced by TRUS were selected and divided into 2 groups; Group I: Patients with chronic prostatitis treated with antibiotic alone for 6 weeks (n = 10), and Group II: Patients with chronic prostatitis treated with antibiotic and prostatic massage for 6 weeks (n = 10). Then the semen analysis, two glass urine test, expressed prostatic secretion test, prostatic culture and sensitivity, and TRUS were repeated for both groups after 6 weeks. The mean number of pus cells in EPS was significantly decreased after treatment in both tested groups. The semen volume was significantly increased after treatment in both tested groups. TRUS findings showed that the diameter of ejaculatory ducts was significantly decreased after treatment in both tested groups. No significant difference between the two modalities of treatment. Pregnancy outcome in patients received antibiotics alone was 20%, while in patients treated with prostatic massage and antibiotics was 10%. In conclusion, TRUS is a good screening diagnostic technique in all patients with low semen volume. Patients with chronic prostatitis induced ejaculatory duct obstruction should be primarily treated with culture-sensitive antibiotics. Prostatic massage adds nothing to antibiotic alone in treatment of EDO resulted from chronic prostatitis.
