Abstract

Iron chelator, is a drug which acts as small molecules that bind very tightly to iron ions to detoxify excess iron from the body and prevent iron mediated injury to body cells in various iron overload disorders.Parentral iron chelators as desferrioxamine (desferal) are poorly absorped from the gut, so should be administered in long daily infusion resulting in poor compliance by the patientsAiming to improve compliance, oral iron chelators have been established for giving good response in removal of excess iron. Deferiprone, is a low molecular weight oral iron chelator. It has a better cardiprotective drug than desferrioxamine, but it should be given several times per day and is rarely associated with agranulocytosis.Deferasirox (exjade,ICL670), is a new oral iron chelator that is given once daily orally. It has an effective role in the reduction of liver iron concentration , However, it is associated with minimal side effects such as (GIT manifestation , skin rash).The efficacy of iron chelation therapy in iron loading disorders have been determined by estimation of serum ferritin, liver iron concentration and cardiac iron. The most recent advances in the monitoring is magnetic resonance images (MRI) of the liver and heart.
