Introduction: Non-communicable diseases are priority diseases that are from the main contributors to the disease burden and at the same time are amenable for intervention strategies. Aim of Work: Endorsement of an acting role of primary health care in prevention, and management of chronic diseases in Egypt. Subjects and methods : Beni Saleh FHU in Fayoum Governorate was assigned to be appropriate place for conducting this study. Recorded patients with targeted chronic NCD were considered index cases to their family members who subjected to screening for these target diseases. All index and discovered cases were subjected to assessment for their risk factors and intervention part of the study. Results: the current role of assigned FHU concerning screening, early detection of these diseases, health education and counseling was totally neglected. Screening of household members of 126 recorded patients with diabetes, hypertension, combined were 37 cases with diabetes (21.6%), hypertension (62.2%), combined disease (16.2%). Evaluation of study intervention showed significant change in the dietary habits regarding reduction of fatty, sugary and salty food (p <.05) and increased frequent Fruits and vegetables daily intake. The percent of well physically active increased from 4.5% to 57%. BMI, SBP, DBP and FBS measurements significantly decreased. Conclusion: The present study revealed shortage of PHC facilities in CDM especially early detection and counseling activities that need to be properly integrated in PHC facilities.
