Between April 1998 and April 2002, fifty patients with documented lower cervical instability associated with degenerative or cervical spine trauma requiring anterior surgical fixation were studied at Haram hospital, Ministry of Health. The patients were classified as following:-10 patients subjected to plating after trauma to the lower cervical spine.-10 patients subjected to plating after cervical anterior discectomy due to degenerative causes.-10 patients subjected to cage fusion due to degenerative causes.-10 patients subjected to bone cement fusion due to degenerative causes.-10 patients subjected to autogenous bone graft due to degenerative causes.From this study, we recommend the following:1-In one level cervical degenerative disc disease with unstable cervical spine we recommend to do discectomy with bone graft or bone cement.2-In two level cervical degenerative disc diseases with unstable cervical spine and in traumatic cervical spine, we recommend to use locking plate with autogenous bone graft.3-There is no absolute indication for cage insertion but it may be easier and perhaps in old patients with osteophytes and kyphotic curve.
