Although structurally not related, cyclosporine A (CsA) and FK506 belong to a family of immunophilin-binding drugs, each drug binding to its respective cytosolic immunophilin, CsA to cyclophilin and FK506 to FKBps respectively. In our study we tried to predict the sensitivity and the response of the lymphocytes in MLCs to calcineurin inhibitors (CsA and FK506) by comparing the level of IL-2 secretion in MLCs by ELISA. Fifteen (15) chronic renal failure patients were scheduled for renal transplantation. They were followed up for 6 months after transplantation by serum creatinine as a valuable indicator of long term outcome. We found that, the degree of IL-2 inhibition in MLCs was statistically significant on adding FK506 or CsA in their half concentrations and more apparent in their full concentrations. Also we found that, there is no statistical significant difference between the inhibitory effect of FK 506 and CsA on IL-2, weather the drug used in its full concentration or half concentration. Also we didn't find any significant change in the serum creatinine throughout the six months of post transplantation.Also we were not able to find any significant relation between the sex of both recipients and donors; and the age of both donors and recipients; and the degree of IL-2 inhibition in different MLCs.
