Seizures are common neurological problems. Complex partial seizures of temporal lobe origin represent the most common type of refractory epilepsy and proper management necessitates proper localization of the seizure focus. The aim of this study was to assess the value of interictal diffusion MRI in localization of seizure focus.The study was conducted on 30 Egyptian patients presenting with temporal lobe epilepsy (TLE) with a mean age of 27.3 ± 9.7 years. They were subjected to through clinical assessment, conventional interictal EEG, neuropsychological assessment (RCFT), MRI of the brain and interictal diffusion MRI of the brain.Results: the sensitivity of diffusion MRI at b factor 1000 and b factor 500, ROCFT and EEG in lateralizing the epileptic focus was (100%), (70%), (86.6%) and (33.3%).Conclusion: Interictal diffusion MRI proved to be a highly sensitive tool for the precise localization of the seizure focus in TLE patients.
