Complex foot deformity is one of the most challenging conditions in Orthopaedic surgery. It is described as a foot with multiplanar deformities with or without shortening of the foot. It includes relapsed or untreated cases, which can be a complicated by other problems. Traditionally, correction of these deformities can be achieved by extensive soft tissue releases, osteotomies or arthrodesis with or without internal fixation. This usually involves excision of large appropriate bony wedges and has many disadvantages, including neurovascular injury, soft tissue problems and a shortening of the already short foot or even shorter. We present our experience in correction of 20 untreated or relapsed feet deformities in 20 patients managed by gradual correction using simple Ilizarov frame without soft tissue release or bony procedures. Mean age 6.8 years (range 2.5 to 15). A detailed description of the pre and postoperative assessment (using modified functional rating system score), operative technique, postoperative care and follow up along with complications are also presented. The average fixator time utilized for correction and maintenance was 14.1 weeks (ranging from 12 to 16). This was followed by 6 weeks of short leg cast. The patient followed in custom made shoe. The average follow up period was 23.8 months (Ranging from 20 to 28 months). 16 feet of our series (80%) have got satisfactory results, while 2(10%) were rated as fair, and 2(10%) were rated as poor. The Ilizarov technique versatility in foot deformity correction enables correction of individual components of the deformity at rates that may be tailored to achieve accurate three-dimensional control using an easy to handle, light, cheap, and simple frame.
