Role of inflammatory mediators in idiopathic generalized tonic clonic epilepsy

Abstract

Background: Seizure activity itself can induce an inflammatory response by activation of microglia and release of cytokines. This could activate inflammatory pathways leading to an acute BBB dysfunction and neuronal hyper excitability, interfering with the seizure threshold. We analyzed cytokines levels in patients with idiopathic generalized tonic- clonic epilepsy and correlated it with various parameters.Materials and methods: Serum samples from 45 patients with idiopathic generalized tonic clonic epilepsy were prospectively collected in the immediate post-ictal phase, a second sample was collected during the seizure-free period. The serum cytokines levels [TNF-α & IL 2] were assessed (ELISA) in these patients and 45 matched healthy controls. IL 6 CSF analysis was carried out during post- ictal period.Results: As compared to controls, the patients showed elevated serum post ictal (IL 2 &TNFα) levels (p value <0.001). Analysis during the seizure-free period revealed a decrease in serum cytokine levels but still higher than control (p value <0.001& 0.031 respectively). CSF post ictal IL-6 was elevated in patients (p value <0.001).Positive correlation was found between serum postictal TNFα level and frequency of epileptic attacks (P value <0.001) (r= 0.64). A statistically significant association was found between number of AED and serum postictal TNFα results (P value = 0.012). Conclusions: We found increased post-ictal serum (IL 2& TNFα) &CSF (IL6) cytokines level in patients with idiopathic generalized tonic clonic epilepsy.
