Abstract

Objectives: The aim of this study was to assess the levels of Toll like receptors 2 and 4 (TLR2 and TLR4) in the gingival crevicular fluid (GCF) and blood from controlled type 2 diabetes mellitus (DM) patients with chronic periodontitis and subjects with chronic periodontitis alone. Materials and Methods: This study was performed on a total of 75 patients divided into 4 groups; 20 patients with controlled type 2 DM suffering from chronic periodontitis, 20 patients having uncontrolled type 2 DM and suffering from chronic periodontitis, 20 patients suffering from chronic periodontitis alone and 15 healthy control individuals. The levels of TLR2 and TLR4 were measured in the GCF and blood of all patients and controls by ELISA Kits and correlated with the clinical parameters.Results: A significant difference resulted between the tested groups at p≤0.001. Group 2 showed the highest mean GCF and serum TLR-2 and TLR-4 followed by group 1 and group 3 with an insignificant difference between group 1 and group 3. The lowest significant mean GCF and serum TLR-2 and TLR-4 resulted for group 4 compared to other groups. With respect to the clinical variables, there was a positive correlation between the level of TLR-2 and TLR-4 in GCF and serum and PI, GI, PD and CAL of all groups. Conclusion: Both Hyperglycemia and chronic periodontitis influence the expression of TLR-2 and TLR-4 in GCF and serum and this could contribute to a stronger inflammatory response, leading to periodontal disease progression. The expression of TLR-2 and TLR-4 is positively correlated with the severity of periodontal disease.
