The present work aimed to measure the levels of telomerase activity in chronic hepatitis, cirrhosis and hepatocellular carcinoma. Sixty patients and twenty normal subjects were included in the present study. The patients were divided into three groups. Group one included chronic hepatities patients (9 females and 11 males) . Group two included cirrhotic patients (8 females and 12 males) . Group three included hepatocellular carcinoma patients (5 females and 15 males) . The control group included 10 females and 10 males. Patients and controls were subjected to full history taking, complete clinical examination, laboratory investigations including the assay of telomerase enzyme activity by telomeric repeat amplification protocol TRAP) using PCR. Telomerase enzyme was significantly higher in patient groups than in control group. Telomerase activity was high in hepatocellular carcinoma 14/20 (70%) , cirrhosis 7/20 (35%) group but was low detected in chronic hepatities 2/20 (10%) group. Telomerase enzyme is mild in hepatitis, moderate in cirrhosis and high in hepatocellular carcinoma.
