Abstract

Aim of Work: To analyze the impact of HLA tissue typing on the outcome of liver recipients who underwent LDLT and the impact on survival of patient.Patients and methods: It is a retrospective study of 50 consecutive adult male recipients (>18 y) who underwent LDLT at Dar Al-Fouad hospital in Egypt. The outcome was analyzed and correlated with the HLA tissue typing. We have 2 groups; group I 9(18%) recipients with +ve HLA tissue typing and group II 41(82%) recipients with -ve HLA. All preoperative and postoperative data are included with the data of post transplant follow up.Results: Total hospital stay,ICU stay and graft function were similar in both groups; all patients received adequate graft size with appropriate GRWR and showed similar incidence of complications with no significant difference in number of deaths or 5 years patients' survival. HCV recurrence was more frequent in positive cross matched group.Conclusion: HLA +ve cross match is not a contraindication for LDLT and further study is required to clarify the correlation between HLA +ve cross match and HCV recurrence post LDLT and to determine the possible correlation between immune tolerance and +ve cross match.
