Abstract

Primary health care (PHC) can be considered the first contact between the patient and the health care system. Objectives: To assess the different aspects of primary health care services, assess their satisfaction and assess customer satisfaction level. Subjects and methods: This is a cross sectional health service research carried out at seven health facilities ( two family health centres and five family health units) in Qaliubiya governorate( were selected systematic randomly) all of them were accredited. Assessment of service delivery in the facilities was done according to Egyptian accreditation standards. The data was collected by questionnaire method. The study sample consisted of 172 medical service providers (physicians, pharmacists, nurses) who completed the questionnaires and 140 clients who filled another interview questionnaire. Every one of the clients fill one questionnaire which assesses the services in general and some of them fill another questionnaire regarding specific services (Diabetes Mellitus(D.M) and hypertension management - antenatal care – Integrated Management of Child Illness(IMCI) service –family planning service) they may have been obtained from the health facility. SPSS software was used for statistical analysis.Results: Scores ranged from 80.3% achieved by Almarkez altebi to 72.2 % which was achieved by Kafr tohoria family health unit. Among the seven dimensions of accreditation the researcher measured, quality improvement program achieved the highest score 87%, while the dimension of the patient rights had the lowest score 67.7%. It was noted that the main general weakness points in these facilities were lack in the number of physician, shortage in drug availability, lack in infection control materials and deficient laboratory services. Assessment of provider satisfaction reported that most of them complained from low salaries, lack of variable types of drugs, lack of training courses and most of them encourage applying paid family medicine services with fund. Assessment of service delivery and client satisfaction revealed that some basic services (measurement of temperature and blood pressure, physical examination, treatment prescription and educational messages) had not met the clinical guidelines (CGs) standards. Less than half of the clients reported that services did not meet their expectations; however more than half of them reported that if they had any health problem they will come to these facilities again. Conclusion: It is concluded from the current study that PHC facilities need more number of family physicians (with good distribution of them among facilities) and more supplies. Providers need more training programs based on CGs.
