The aim of the work is to evaluate the effect of repeated intracavernosal self-injection of vasoactive drugs in patients with elevated End Diastolic Velocity (>5 cm/sec) during pharmacopenile duplex ultrasonography (PPDU).This study was carried out on group of patients complaining of erectile dysfunction recruited from Andrology clinic, Kasr El Aini hospital, Cairo University who were diagnosed as venous leakage erectile dysfunction by pharmacopenile duplex ultrasound, their age ranged from 23 to 65 years old, 21 were smokers, ten were diabetic, two were hypertensive, one has history of cerebral stroke, four showed low total testosterone level and two showed high prolactin level in the serum.Duplex evaluation was performed to the patients on self-injection therapy for comparison of end diastolic velocity and resistive index before and after finishing of the eight doses of IC self-injection.According to the color duplex ultrasonography, there was significant decrease in the end diastolic velocity (<5) in ten patients including six that could achieve erection sufficient for satisfactory intercourse after eight doses of repeated IC self-injection therapy in comparison to it before starting the treatment course, It was noted that the age is the most important factor that may contribute in improving the erectile response to ICI as younger patients show better response, On the other hand there were many factors (e.g. smoking, diabetes, hypertension, TTE & PRL levels)that did not show that significant effect on the erectile response to ICI.Early rehabilitation of the patients with venous leakage ED using ICI may help to regain normal erection and avoid to be underwent penile prosthesis.
