Diastolic dysfunction is common after cardiopulmonary bypass after coronary revascularization. Milrinone is a phosphodiesterase enzyme type III inhibitor which increases intramyocardial levels of cyclic adenosine monophosphate, which, in turn, increase contractility and relaxation. The administration of Milrinone in this patient population was associated with increased the cardiac output, decreased the preload and systolic blood pressure without chronotropic effect.Milrinone improves the left ventricular relaxation using the most specific indices (color M-mode and Tissue Doppler) immediately after drug administration and till chest closure.
