Testicular maldescend in one of the most common clinical entities encountered in pediatric surgery and it affects approximately 5% of all full male infants and 20% of all premature male infants. In general the overall incidence of cryptochidism in the general population is approximately 0.8%, making it the most frequent disorder of sexual differentiation in male patients.The standard management of cases of impalpable testis include, localization measures (e.g. sonar), hormonal therapy and surgical approach.The advent of laparoscopy in the management of impalpable testis was evaluated in 50 cases, 43 cases were unilateral and 7 cases were bilateral (total of 43 + 14 = 57) their age ranged from 14 months to 6 years. It was found that laparoscopy could evaluate the site and volume of the testis as well as the condition of the spermatic vessels in all cases (100%) in addition, biopsy of atrophic tests (31.6%) could be done or assisted by laparoscopy.Various forms of orchiopexy (one stage, staged, Fowler Stephens technique) could be achieved laparoscopically in all cases with viable testis(68.4%).
