Intraocular pressure in the low teens best preserves vision in patients with glaucoma. The main determinant of long-term IOP after glaucoma filtration surgery is the healing response. Antimetabolites are the most commonly used agents in glaucoma surgery for the modulation of wound healing. However, they are associated with blinding complications. Several other mechanisms are available to modulate the wound healing including minimizing trauma, antagonists to growth factors, matrix metalloproteinase inhibitors, anticross-linking agents (e.g. beta-aminopropionitrile), stimulants of apoptosis.
