MR imaging detects myelomatous involvement of vertebral bodies significantly more often than suspected from plain radiography MRI is important in detection of complications and in follow up of cases after treatment especially non secretory myeloma. This study describes the different infiltration patterns of multiple myloma involving the spine in 40 cases using different MRI sequences. Gradient echo and STIR sequence were useful techniques in the detection of spinal infiltration, also the post contrast study (combined with fat suppression) was useful especially in the evaluation of follow up cases after treatment. MRI was useful in SBP and helped greatly in assessment of their conversion to MM.Therefore, like laboratory parameters, biopsies and radiographs, MR imaging can be a supporting pillar in staging and planning treatment of patients with multiple myeloma.
