Abstract

Burn trauma is one of the disastrous health problems to the affected victims and their families. Residual deformities and disfigurements left after deep burns affect the patients lives.One of the disfigurements is burn scars in the face and/or neck.In addition of having physical implications they produce psychological troubles affecting patient daily life.Many patients seek reconstruction of post burn scars in the face and/or neck in order to have better functional and aesthetic appearance .In this thesis a method of reconstruction was used on ten patients as a surgical solution to the scars . It is a combination of debulked perforator flap and tissue expansion use, to gain the benefits of both. It depends upon insertion of tissue expander underneath the normal skin of the adjacent region nearby the scar and followed by elevation of a perforator dependant cutanous flap and then fashioning and debulking of the flap to fit and reconstruct the defect left after scar excision .Follow up of the patients revealed improvement of the appearance and replacement of the scars by a highly matched skin. Some complications occurred and handled easly with cooperation of the patientsThe overall results were satisfactory and the aesthetic outcome regarding the colour match, contour and skin elasticity were more pronounced with lesser donnor site morbidity than other surgical procedures.
