Spontaneous abortion is the most common complication of pregnancy and is responsible for significant emotional distress to couples desiring children.Habitual abortion was classically defined as the occurrence of two or more consecutive clinically recognized pregnancy losses before 20 weeks from the last menstrual period.Objectives: To evaluate uterine cavity pathology in cases of habitual abortion using office hysteroscopy. Patients and methods: The present work was performed on a group of thirty patients in the reproductive age with a history of two or more consecutive spontaneous 1st&2ndtrimesteric abortion attending the obstetrics and gynecology outpatient clinic in Kasr El Aini Hospital, Cairo University.All patients were subjected to careful history including obstetric history, menstrual history, past history of any investigations or treatment, gestational age of every abortion and through gynecological examination. They were subjected to office hysteroscopy and hysterosalpingography.This study was done in the post menstrual periods for evaluation the uterine cavity pathology in cases of habitual abortion using office hysteroscopy.Results: 18 women (60%) had a normal hysteroscopy . The remaining 12 women (40%) presented structural uterine anormalies: 4 patients (13.3%) had intrauterine adhesions, 2 (6.7%) had submucous myomas, 1 (3.3%) had polyp and 5 (16.7%) had congenital structural uterine anomalies (three cases of spetate uterus (10%) and two of bicornuate uterus (6.7%) .Compared to Hysteroscopy as gold standard, hysterosalpinography had a sensitivity of 83.33%, specificity of 100%, positive predictive value of 100%, negative predictive value of 90% and overall accuracy of 93.3%.Conclusion: The results obtained here suggest that the uterine factor has high prevalence in patients with a habitual abortion history,and for this reason it should be systematically assessed in patients with a habitual abortion.
