Ankylosing spondylitis is a chronic systemic inflammatory disorder that affects primarily the axis skeleton (Khan, 1997).The involvement of proximal large joints (hips or shoulders) has been reported in one series to affect 50% of patients with severe as seen in hospitals (Calabrio, 1987). This study is planned to find out the prevalence of clinical and radiological shoulder involvement in As patients and its relation to other disease parameters. This study included twenty male patients diagnosed an As according to the (modified new York criteria, 1984), and ten healthy males with matched age as a control group. All patients had been subjected to full history taking and full clinical examination including full detailed clinical and functional assessment using ankylosing spondylitis assessment question nave (ASAQ) and arthritis impact measurement Scales (AIMS). Radiological performed to all patients and controls. Thirteen patients (65%) had clinical shoulder involvement 12 (60%) had both pain and stiffness and limited range of active shoulder motion, one (5%) had only limited age of shoulder 10 (50%) had pam and stiffness in both shoulder, 2 (10%) had pain and stiffness in the Lt. Shoulder. Only nine patients (45%) had limited range of motion in both shoulder and 3 (15%) and shoulders, respectively. Thirteen patients (65%) had radiological was found between radiological shoulder involvement in our patient and the controls (P>0.05). A statistically right significant relation was found between limitation in the range of motion and radiological in the left shoulder (P>0.05). A non-significant relation was found between radiological shoulder involvement and other disease parameters.
