Fibromyalgia syndrome is a chronic pain syndrome characterized by widespread musculoskeletal pain and multiple tender points at specific anatomic sites and other features according to the multicenter criteria of the American College of Rheumatology (ACR) which are: sleep disturbances, morning stiffness, fatigue, paresthesia, anxiety, headache, and irritable bowel. Fibromyalgia syndrome is one of the commonest rheumatic. However, the pathogenic mechanisms are unknown. There is a tendency to localize the primary disorder underlying fibromyalgia in the central nervous system. Endocrine regulation is considerably impaired, with many hormonal mechanisms altered. Therefore, neuro-mediator and hormonal abnormalities may play an important role in pathogenesis of fibromyalgia as serotonin, substance P, growth hormone. Although pain and tender points tend to cluster around the axial skeleton in patients with fibromyalgia, pain and paresthesia in the upper limbs are common complaints in these patients. As carpal tunnel syndrome is the most common peripheral entrapment neuropathy many studies has been done to estimate the prevalence of carpal tunnel syndrome in patients with fibromyalgia. In the most recent study, the prevalence was 10%.
