This study aimed to evaluate the nano DCD/CaP/DAE surface implant osseointegration in mandibular bone (bone density) after three and six months using cone beam computed tomography (CBCT) and to evaluate the bone marginal bone height loss, plus measuring the implant stability using the resonance frequency analysis (RFA) (by using Osstell device) and compare it to TPS. This clinical study was done on six patients, each patient was selected to restore their missing teeth; each patient received two different types of implants in the same side of the posterior mandibular area; the discrete crystals deposition nano calcium phosphate surface with dual acid etched implant (DCD/ CaP/ DAE) and the titanium plasma spray surface type implant (TPS).The latter is consider as a self control group. According to implants surfaces, the implants were divided into two main groups:Group (I): This group included six nano DCD/CaP/DAE implants.Group (II): This group included six TPS implants.Data was recorded and statistically analyzed. The results showed that there was a high significant difference between both groups in term of bone density around implant after three and six months, which showed increasing in Hounsfield Unit (HU) in nano (DCD/CaP/DAE) type more than TPS type, according to the readings of the cone beam computed tomography. After six months, there was a significant difference between both groups in terms of implant stability, which showed more stability in the nano type implant, than the TPS implant according to the readings of the Osstell device. There was no significant difference in marginal bone height loss between the two groups between the 3rd to the 6th month after implant insertion. Conclusion: Nano DCD/CaP/DAE implants are superior in bone enhancement than the TPS implants. Although the TPS system was much easier in tools of implant insertion and drills were sharper than the nano type system.
