The current study contained 12 spectra screw plant immediately inserted in healthy patient after extraction. At the same time provisional crowns were inserted in place to restore esthetic. All patients included in this study have no contra-indication for surgical intervention. Patients were instructed to take antibiotic (Amoxicillin 1 g every 12 hours) one day before implantation and 4 days after implantation procedure in order to mentain plasma antibiotic level high. The badly decayed teeth were extracted gently to preserve the alveolar bone with proper irrigation of extraction site, then implants were inserted in place with hydroxyapatite powder implants were prepared, impressions were taken and provisional crown cemented in place. The patients classified into two groups:First group provisional crown were held in contact with opposing teeth (immediate functional loading), while second group provisional crown were held out of contact (delayed functional loading).Evaluation of the implant was done using digital radiograph at 1,3and 6 month post-operatively.In all cases complete bone formation around implant was reported for both groups. Two ways ANOVA reveals a significant increase in bone density for both groups. On the same time increase bone height also for both groups and this increase was significantly with the use of delayed functional loading compared with immediate functional loading.
