Purpose: This study aimed to evaluate clinically and radiographically the efficacy of both mini-plates and lag screws on the stability of fixation of the anterior mandibular fractures. Patients and Methods: A total of 12 patients diagnosed with symphyseal or parasymphyseal fracture. The patients were divided randomly into two equal groups six patients each according to the technique of internal fixation used for their treatment as follows: Group A : patients underwent open reduction and internal fixation (ORIF), using two lag screws and Group B : patients underwent ORIF using two miniplates. Follow up of cases was done clinically and radiographically using cone beam computed tomography at usual postoperative intervals with shortest long term long term follow up of two months. Results: The data from all patients proved that both techniques are predictable as regard to stabilization of the fracture segments, uneventful healing without any sort of infection. Conclusion: Both lag screws and miniplates osteosynthesis are predictable techniques for fixation and stability of anterior mandibular fractures. Also there were no statistical significant differences regarding all the estimated criteria between lag screws & miniplates in the current study regarding the stability of the fracture segments.
