Background: Exploratory laparotomy is essential in the care of abdominal trauma. However, negative laparotomy has a significant morbidity rate. Various Adjunct methods (FAST, CT, LWE, and DPL) help to identify patients who are unlikely to benefit from laparotomy. The initial reason for incorporating laparoscopy into trauma was actually to diagnose injury and reduce unnecessary, non-therapeutic procedures. Various studies showed usefulness and safety of laparoscopy in trauma setting in haemodynamically stable patients. . Laparoscopy allows early and safe discharge of patients. Laparoscopy has the advantage that it can be used as a therapeutic tool as well. It is the modality of choice for assessment and treatment of suspected diaphragmatic injuries .Methods: We present a case series of 60 patients who fulfilled the preset criteria during the period of study. Patients were managed in Kasr El Aini Hospital, Cairo University Hospitals and Imbaba General Hospital between July 2011 and September 2012. Local wound exploration was done to 58 patients as two patients presented with omental herniation, patients with proved peritoneal violation or equivocal local wound exploration were subjected to laparoscopy. Results: Local wound exploration revealed no peritoneal violation in 27 patients (45%).25 patients (41%) had either a negative, non-therapeutic or therapeutic laparoscopy). Laparotomy was avoided in the two previous groups (86%).This allowed prompt discharge of this group. Therapeutic laparoscopy was possible in 6 patients. This involved: repair of diaphragmatic injuries, repair gastric tear and control of source of haemoperitoneum. Laparoscopy was converted to laparotomy in 8 patients out of 33 patients subjected to laparoscopy. There were no significant complications related to laparoscopy or to general anesthesia. There were no missed injuries by laparoscopy requiring re-intervention. Conclusions: Benefits of trauma laparoscopy are indisputable in equivocal penetrating wounds and diaphragmatic injuries.. Laparoscopy can decrease significantly the rate of non-therapeutic and negative laparotomies with their related morbidities. It is increasingly used with a therapeutic intention.
