تعتبر الرعاية التلطيفية نوع من أنواع الرعاية الصحية والتي تقدم للمريض وللقائمين على رعايته من ذويه بهدف الحد أو التخفيف من معاناتهم في مواجهة المرض، تستهدف تحسين نوعية حياة الأفراد الذين يعانون من مرض مزمن، حيث يتم تقديم خدمات الرعاية التلطيفية من خلال فريق متخصص يسعى إلى تحقيق التكامل في تقديم الخدمات للمرضى، ويعتبر الاخصائى الاجتماعى أحد أعضاء فريق الرعاية التلطيفية، وقد استهدف هذا البحث تحديد المعوقات التي تواجه فريق الرعاية التلطيفية عند التعامل مع مرضى القلب، والتوصل إلى برنامج مقترح من منظور الممارسة العامة في الخدمة الاجتماعية للحد من المعوقات التي تواجه فريق الرعاية التلطيفية عند التعامل مع مرضى القلب.
