Transfixing the Boxer’s Fracture by K.wires to the Adjusent Healthy Metacarpal: Prospective Randomized Study

Abstract

Background: The neck fractures of the little finger metacarpal are common and mainly occur in young men. The aim of this work was to evaluate the clinical and radiological outcomes after closed reduction and fixation of fifth metacarpal neck fractures (Boxer 's fracture) using transverse K-wires fixation. Methods: This prospective randomized study was conducted on 20 isolated fracture of 5th metacarpal neck patients were treated by percutaneous transfixing K-wires, aged equal or older than 15 years old, males only. All patients were subjected to: Routine preoperative investigations, history taking, clinical examination [general examination, local examination (The rest of the hand skeleton, (checking the initial deformity (angulation, rotational malformation), skin condition, the intensity of the edema all over the hand)],and radiographic evaluation [Standard anteroposterior, lateraland oblique X-rays were done]. Results: The Grip strength of studied patients ranged from 75-96 %. The Quick DASH disability Score of all studied patients was zero except one case had scores (20, 25). All patients were satisfied and resume their work and daily activity except one patient; despite fracture union, he was unsatisfied due to partial stiffness and residual pain at the hand with manual activities. The TAM score of studied patients ranged from 220 - 270 degrees. Conclusions: The utilization of transfixing Kwires for fixation may facilitate prompt hand mobilization, repair the deformity, and yield favorable clinical and radiographic results.
