Pattern of Serum Osteopontin Level in Patients with Cirrhotic Liver versus Patients with Hepatocellular Carcinoma

Abstract

Background:The overall prognosis of HCC is generally poor, mostly due to the difficulties of early diagnosis. Therefore, it's very important to identify new, highly sensitive, and specific markers for HCC to enhance the early detection rate and improve treatment effects. Several reports have proposed a valuable role for osteopontin (OPN) as a promising biomarker for early diagnosis of HCC. Aim:To evaluate the pattern of OPN in Egyptian HCC patients versus cirrhotic patients. Subjects and Methods:This comparative study was conducted at the Internal Medicine department in Suez Canal University (SCU) hospital, Ismailia, Egypt. Ninety individuals were enrolled in our study and assigned into three groups. The HCC group included 40 patients with HCC, the cirrhotic group included 40 patients diagnosed with liver cirrhosis mostly due to HCV, and the control group included 10 apparently healthy individuals. We compared AFP and OPN levels between the three groups. Results: Mean serum OPN and AFP levels were significantly higher among HCC patients, compared to cirrhotic patients and the controls (P<0.001). Moreover, OPN was positively correlated with AFP (P<0.001) and bilirubin level (P<0.001), whereas it was negatively correlated with albumin level (P<0.001). Meanwhile, OPN was not significantly correlated with AST or ALT (P= 0.061 and 0.102, respectively). Conclusion:OPN level is significantly elevated in HCC and cirrhotic patients, and it's positively correlated with AFP and bilirubin, and negatively correlated with albumin levels. In HCC patients particularly, OPN levels decrease significantly after treatment.
