Role of Serum Prolactin in Differentiating Tonic Clonic from Psychogenic Non-Epileptic Seizures in Emergency Department-Suez Canal University Hospital

Abstract

Background:As paroxysmal events resemble epileptic seizures but lack electroencephalogram correlation, psychogenic nonepileptic seizures are a type of conversion disease. Prolactin (PRL) is a polypeptide that lactotrophic cells secrete during epileptic seizures; prolactin release from hypothalamic stimulation may be connected with the propagation of epileptic discharges.Aim:To assess the ability of serum levels of serum prolactin to differentiate between Generalized tonic-clonic seizures (GTCS) andPsychogenic nonepileptic seizures (PNES).Subjects and Methods:Twenty GTCS patients and twenty PNES patients who visited Suez Canal University Hospital were the subjects of our prospective cohort research. The ABCDE technique was used to clinically evaluate and manage patients. Each patient underwent an initial evaluation that took into account their medical history, clinical examination, laboratory investigation, and radiographic evaluation. Blood samples of serum PRL were taken from patients in both groups two hours after the event and 24 hours later.Results: The current study found statistically significant older age with psychogenic nonepileptic seizures compared to GTCS with p-value=0.037, while there was no gender difference between both groups, no statistically significant difference was found between GTCS and PNES as regards serum Na and K levels with p-value >0.05, 75% of patients with GTCS had positive EEG findings while only 10% of PNES had positive EEG findings. Serum PRL was significantly higher among GTCS than PNES with a p-value <0.001.Conclusion: Generalized tonic colonic seizures and psychogenic non-epileptic seizures can be distinguished using serum prolactin.
